Civil partnership notice forms

A civil partnership notice form helps the Registrar collect information about your ceremony and prepare your details for registration.

Without it, a civil partnership cannot go ahead.


Foreign divorce form

If you have been divorced outside of the UK or the Republic of Ireland, you must fill in a foreign divorce form and return it along with your civil partnership notice forms.


Civil partnership in the Registrar's office

This brochure includes details of the ceremony format and information about dress, music and flowers.

It also contains a form, describing your choice of format, which should be returned to the Registrar no later than two weeks before your ceremony date, along with any necessary documents or equipment.
If you want to have your ceremony in the Registrar's office, you must complete the following form and return it to the Registrar at least two weeks before your ceremony, along with any outstanding documents.

This form is for venues who wish to apply for a licence to hold civil marriage and partnership ceremonies on their premises. It is valid for three years.
